Becoming a Member
Who Can Apply?
The DET OHT welcomes applications from organizations interested in participating in one of the following Working Groups for our ongoing projects:
- Community Member Engagement Working Group
- Health Equity and EDI Working Group
- Mental Health and Integrated Pathway Working Group
- Performance, Measurement, and Quality Improvement Working Group
- Digital Working Group
In addition to Working Group participation, the following criteria must be met by the applicant organization to be eligible for admission as a Team Member in the DET OHT:
- Deliver services within the catchment area or attributed population of the DET OHT
- Uphold the values outlined in the Patient, Family and Caregiver Declaration of Values
- Have a demonstrated commitment to equity, as defined by Ontario Health
- Commit to the work of the Ontario Health Team, including the vision, guiding principles, roles and responsibilities outlined in the Collaboration Decision Making Agreement
- Commit resources to achieve the vision and goals of the DETOHT (e.g., time, expertise, staff, space, communications, money)
- Commit to acting as an ambassador and advocate for the work and activity of the OHT
- Comply with all OHT deliverables and obligations
- Receive some public sector funding to deliver its services
How and When to Apply?
The Fall 2024-25 membership application is now closed. If you are interested in becoming a Team Member, please stay tuned for updates on our next application round for 2025-26.
Applicants will be required to complete an Expression of Interest (EOI) form, which will ask organizations to outline the following:
- Name and Contact for the Signed Team Member Organization
- Confirmation and description of how the applicant meets the eligibility criteria
- Articulation of how the applicant supports DET OHT Strategic Plan
When Should Applicants Expect to Hear Back?
All applicants will receive confirmation of receipt of their application, followed by a response with the final decision, in writing, in February 2025.
Successful applicants will be on-boarded and introduced at the DETOHT Team Members meeting in April 2025.
