Current Initiatives

Lower Limb Preservation
An initiative with the goal to establish care pathways that offer specialized primary and community care focused on foot and wound care for individuals with diabetes or peripheral artery disease.

Past Initiatives
Digital Shared Care System
This pilot aimed to explore the secure digital exchange of patient and client information between a health information custodian (HIC) and a non-HIC. The goal was to ensure compliance with privacy legislation while addressing care gaps, improving efficiency, and enhancing the overall care experience.

Emergency Department Outreach Worker Program
To support patients at the St. Michael’s Hospital Emergency Department with challenges beyond health, this program assigned a dedicated outreach worker to those experiencing deep poverty.

Drop-ins Service Provider Team Pilot Project
The DET OHT collaborated with the Toronto Drop-in Network to bring low-barrier access to health services for people at the drop-in centres (housing shelters) in Downtown East Toronto.
