Privacy Policy
The DET OHT respects and upholds the privacy and confidentiality of all community members, patients, families, and caregivers. DET OHT ensures that it safeguards patient and client information in accordance with the Personal Health Information Protection Act (PHIPA) and other applicable laws.
Data Sharing
As part of this commitment, DET OHT will ensure protecting patient and client information using the following strategies:
- For evaluations, research, and surveys, only de-identified patient and client data will be used. If research involves personal health information, explicit consent from the individual will be obtained.
- Information collection is limited to what is necessary for these purposes.
- No personal health information will be accessed, used, modified, copied, destroyed, or disclosed to external parties outside DET OHT, except when necessary for patient care and with patient consent, or as otherwise permitted or required by law.
- All members of the DET OHT, including those in Operations Team, Core Group (board members), and its member organizations, are bound by confidentiality agreements.
- Patients and clients have the right to withhold or withdraw consent as appropriate.
- Any individual or organization participating in DET OHT initiatives must confirm their commitment to the privacy and confidentiality before accessing any patient or client information.

DET OHT Programs and Services
DET OHT’s programs and services aim to enhance coordinated care and improve patient experiences. Some key initiatives include:
- Interprofessional Primary Care Team, an initiative dedicated to increase access and attachment team-based primary care for individuals experiencing significant financial and social barriers, particularly those from Black and Indigenous communities. Learn more.
